Optional Insurance Coverages

 
Employers can choose to have these optional benefits available for their employees. Employees must enroll and remain enrolled in the Health Indemnity Plan to be eligible to participate in the following optional coverages:
 

 


Term Life/Accidental Death and Dismemberment (AD&D) Insurance - Sample Plan

 (The AD&D benefit applies to the employee only, in an amount equal to the life benefit.)

* For persons age 65 or older, the Death Benefit will be 65% of the amount shown.
** At no time may a covered Dependent's amount of Life Insurance exceed 50% of the Employee's Amount. Any necessary reduction in a Dependent's amount of coverage will be effective on the same date that the Employee's Amount reduces.


Short Term Disability Insurance (where available) - Sample Plan

Maximum Disability Period (Accident or Sickness): 26 weeks

Elimination Period (Accident or Sickness): 14 days

Benefit Amount: 50% of pay up to $150 per week

Minimum Disability Benefit: $50 per month

Maximum Mental Illness Disability Period: 3 months (including hospital confinement)

Maximum Alcoholism/Drug Addiction Period: Up to 15 days in any 12-month period

* Based on usual and customary charges.

** Major Restorative Services will not be covered until the covered person has been continuously insured under this dental plan (or the dental plan replaced) for 12 consecutive months.
